Lots of good ideas already, but on the assumption that you can go whereever you want on your trip, I would suggest that you consider a few of these as well as those already mentioned:
De Dolle Brouwers in Esen, West Flanders - not far from Brugge - one of the best brewery tours for two reasons: firstly Kris's mum who does the English tour, is an amazing woman with many great tales to tell; and secondly the tasting room is unbelievably cosy, especially in December. They are only open Sat and Sun but I would say this is a 'brewery tour that knocks your socks off' and a beer that's a sip of nirvana (anything on draught at the brewery tap) all in one hit.
Watou: stay at the B & B at St Bernardus and go for dinner at 't Hommelhof. This is something that everyone should do at least once in their lives. Beer cuisine par excellence and the best B & B you will ever find - with of course superb beer on site, free courtesy of Madame.
Fantome: go visit Dany way over east in Luxembourg province - and while there you can head south to this superb place to stay:
An amazing hotel right on the banks of the Semois river.
Hope this gives you some idea of the more obscure places to go. Of course, even just going to one of the four best known cities in Belgium - Gent, Brugge, Antwerp or Brussels - you will have more superb bars in each city than you can shake a stick at.
